Job Overview
The Physical Therapist (PT) is responsible for the full range of physical therapy treatments and tasks. Evaluate and treat patients with various disabilities. Designs and monitors therapeutic programs to ensure safe progression toward therapeutic goals. This role is for Inpatient and Outpatient.
Essential Functions:
- Provide appropriate evaluations.
- Provides treatment based on treatment plans.
- Ensures the education of the patient and the family.
- Provides coordination of care. Contact Physician regarding the patient's medical condition.
- Coordinates a schedule and treatment plan with patients to ensure their needs are met.
- Bachelor’s Degree. Completion of a physical therapy course approved by the Council on Medical Education and Hospitals and the American Medical Association in collaboration with the American Physical Therapy Association.
- Licensed as a physical therapist in the State of Wisconsin or eligible for licensing
- Current BLS certification
- Two years experience in Home Health.
- Basic computer competency skills are expected for this position
- Microsoft Office Suite Skills: Beginning Skills – Able to perform basic things such as opening a document, change font, save a document, and write a letter.
