The Physical Therapist (PT) evaluates the physical therapy needs of assigned patients, establishes goals with the patient and plans, develops and implements individual physical therapy programs designed to improve patient mobility and independent function. The PT provides age and developmentally appropriate care to pediatric to geriatric patients. Effectively interacts with patients, doctors and other healthcare members. Responsible for overseeing PTA as requested.

Essential Duties and Responsibilities:

1. Assess, plan, treat, evaluate and implement a POC for all clients seen.
2. Provides supportive supervision of PTA’s and PT / PTA students. ding, financial support & appropriate billing.
3. Practices within the scope of APTA standards and Code of Ethics.
4. Communicates and interacts with patients, families, physicians and staff demonstrating behavior consistent with the hospital’s core values.
5. Promotes a clean and safe environment of care, utilizing the SAFE error prevention habits.
6. Provides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct.
7. Completes documentation of all patient care activities in a timely manner in the electronic medical record.
8. Develop professional skills by defining and completing appropriate Development plans, participating in continuing education and in-service offerings, using written resources to keep current with advancements in the field and seeks out resources with recognized expertise within and outside the Department to enhance quality of care.

Education and Training:

1. Completion of a Master’s or Doctorate degree program from an accredited college or university.
2. Work requires the level of knowledge acquired by completing a Master’s program in Physical Therapy from an accredited college or university.
3. Current CPR certification required.
4. Current license as a Physical Therapist from the State of Illinois.
