Home Health Physical Therapist

Responsibilities:

• Completes appropriate goal re-assessment and discharge planning.
• Performs comprehensive physical therapy evaluations, treatment, and related follow-up services including discharge planning.
• Develops a treatment plan with measurable goals based on patient assessment.
• Collaborates with patient and other healthcare personnel or outside agencies regarding coordination of care.
• Documents patient assessments and pertinent information in a timely manner.
• Provides services and treatments requiring physical therapy skills according to the written plan of care and the signed plan of treatment.
• Supervises Physical Therapist Assistants and Home Health Aides.
• Provides physical therapy according to standards of care defined by the state practice act and other professional and governing agencies.

Qualifications:

• Bachelor's Degree- Physical Therapy - Required.
• 1 Year Physical Medicine/Rehabilitation Experience- Required.
• 1 Year Home Health Experience - Preferred.
• Physical Therapist (PT) State Licensing Board - Required.
• Cardiopulmonary Resuscitation (CPR)- American Heart Association - Required.
• Driver's License - RequiredAuto Insurance - Required.
