Description
Responsibilities:
- Completes appropriate goal re-assessment and discharge planning.
- Performs comprehensive physical therapy evaluations, treatment, and related follow-up services including discharge planning.
- Develops a treatment plan with measurable goals based on patient assessment.
- Collaborates with patient and other healthcare personnel or outside agencies regarding coordination of care.
- Documents patient assessments and pertinent information in a timely manner.
- Provides services and treatments requiring physical therapy skills according to the written plan of care and the signed plan of treatment.
- Supervises Physical Therapist Assistants and Home Health Aides.
- Provides physical therapy according to standards of care defined by the state practice act and other professional and governing agencies.
Qualifications:
- Bachelor's Degree- Physical Therapy - Required.
- 1 Year Physical Medicine/Rehabilitation Experience- Required.
- 1 Year Home Health Experience - Preferred.
- Physical Therapist (PT) State Licensing Board - Required.
- Cardiopulmonary Resuscitation (CPR)- American Heart Association - Required.
- Driver's License - RequiredAuto Insurance - Required.
