Description
Under the general direction of the Home Health Administrator, the Physical Therapist evaluates and treats patients recovering from injury, disease, or surgery in a home care environment. Establishes treatment plans and treats patients using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease patients' pain and help them increase their mobility. Works cooperatively with physicians, nurses, case managers, and adjustors.
Responsibilities:
- Conduct an in-person comprehensive physical therapy assessment on Alivia Home Health patients, as assigned.
- Coordinate with the case management team to develop a comprehensive plan of care for each patient.
- Conduct an in-person physical therapy assessment or reassessment, as needed.
- Participate as a member of a case management team, communicating patient changes, collaborating on care planning decisions, and coordination of 24-hour care delivery.
- Actively participate in the Intake and Assessment meetings.
- Evaluate patients to determine and assure appropriate prescription, needs, goals, history, complications, and precautions.
- Follow-up with the patient during treatment, explain precautions to take.
- Discusses condition and treatment plans and recommends solutions to patients' care problems with the case management team.
- Assess the need for adaptive durable medical equipment, specialized shoes, and prosthesis.
- Assess patient’s home for safety and mobility.
- Recommend home modifications and or equipment.
Qualifications:
- Current Licensure as a Physical Therapist in the state of Florida.
- Minimum of 1-year working as a Physical Therapist preferred however recent graduates and newly licensed PTs are encouraged to apply.
- Current State of Florida driver’s license with driving record acceptable to Alivia Care insurance carrier, and reliable transportation.
- Current Auto Insurance coverage.
- Current CPR certification.
- Level II AHCA (Agency for Healthcare Administration Background Screening).
- Bi-lingual Spanish a plus.
