The Physical Therapist is responsible for assisting persons with physical disabilities to achieve maximal functional level of independence to return to home environment. Must support the mission of providing Sacred Work to all who enter the doors of Baptist Hospitals of Southeast Texas.
Essential Job Duties:
- Manages scheduling of own patient load and other tasks to allow
smooth work flow and effective patient care. Including special projects and miscellaneous duties as needed to promote the smooth work flow of the department. - Evaluates (including re-evaluations) and documents patient's functional abilities and disabilities, range of motion, strength, coordination, etc. through assessment and testing procedures. Establishes functional baseline to be used throughout treatment as a gauge of progress and physical improvements. In accordance with patient's assessment and physician's prescriptions, establishes and documents an individual therapy plan. Modifies treatment plans in accordance with patient's response and condition changes.
- Practices appropriate safety measures in assisting patients with the performance of job related duties/activities. Follows necessary and pertinent safety precautions during the performance of job duties to prevent injury to self, patients, and/or others and to prevent damages to hospital equipment/property. Has experienced no avoidable accidents and has not contributed to any in the past 12 months
- Confers with patients and families to address discharge needs i.e. regimen of home treatment, patient's level of physical independence and other details of required care (training, equipment, referral to other facility or healthcare provider) with documentation in the medical record as indicated. Completes discharge summaries on each patient in a timely fashion, addressing patients' status at initial and discharge assessment, goal attainment, outcome of therapy and discharge needs.
- Reviews patient's progress with other members of the patient care team. Communicates with physician and other appropriate hospital staff to inform or consult regarding patient status, progress and recommended treatment changes.
Qualifications:
- Graduate of an accredited school of Physical Therapy with previous experience in a hospital environment is required.
- Current licensure in Texas as a Physical Therapist or temporary license while eligible to sit for the State Board exam.
