Description
The Physical Therapist evaluates, plans, and administers therapeutic procedures in physical medicine and rehabilitation as prescribed by a referring physician. This role establishes individualized physical therapy goals, provides treatments, and regularly reassesses patient progress, documenting all findings accurately. The Physical Therapist collaborates with an interdisciplinary team to deliver high-quality patient care while adhering to professional standards, maintaining privacy, and respecting patient rights.
Essential Functions:
- Conducts thorough physical therapy evaluations to assess patient needs and develops individualized treatment plans.
- Administers safe and effective therapy treatments, ensuring all interventions are appropriately documented.
- Reassesses and adjusts treatment plans within appropriate timeframes to align with patient progress and changing needs.
- Provides clear, concise, and accurate documentation of evaluations, treatment plans, progress notes, and billing information.
- Educates patients and their families on treatment plans, therapeutic exercises, and post-treatment care to support long-term recovery.
- Operates therapy equipment safely and maintains a clean, organized, and safe environment for patients and staff.
- Collaborates with referring physicians, peers, and interdisciplinary team members to ensure continuity and quality of patient care.
- Demonstrates population-appropriate competencies for all relevant patient demographics, tailoring care to individual needs.
Qualifications:
- Bachelor's Degree in Physical Therapy required.
- Doctorate in Physical Therapy (DPT) preferred.
- 0-2 years of experience in a physical therapy setting required.
