Description
Key Responsibilities:
- Patient Evaluation: Conduct thorough initial assessments of patients' physical abilities, functional limitations, and rehabilitation needs. Utilize standardized tests and measures to establish baseline data and identify goals.
- Care Planning: Develop personalized treatment plans based on assessment findings, patient goals, and physician recommendations. Collaborate with the healthcare team to ensure integrated care across disciplines.
- Therapeutic Interventions: Implement individualized physical therapy interventions, including exercises, manual therapy, modalities, and patient education. Adjust treatment plans as needed based on patient progress and feedback.
- Communication: Maintain ongoing communication with the prescribing physician, nurse practitioner, or physician assistant regarding patient status, treatment efficacy, and any changes in patient condition. Document all interactions and updates in the patient’s electronic health record (EHR).
- Patient Education: Educate patients and their families about rehabilitation processes, home exercise programs, and strategies to enhance safety and mobility. Provide resources for continued care and wellness.
- Functional Training: Assist patients in achieving independence in daily activities through functional training and adaptive techniques. Focus on improving mobility, balance, strength, and coordination.
Qualifications:
- Education: Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) from an accredited program.
- Licensure: Valid Physical Therapist (PT) license in [State].
- Experience: Minimum of [X] years of clinical experience, preferably in home health, geriatrics, or rehabilitation settings.
