Description
The Physical Therapist provides evaluation and sets up treatment programs for all patients referred to physical therapy; provides physical therapy treatment to referred patients; writes and reviews patients progress reports, including charting, and reports to referring physicians.
Responsibilities:
- Documents evaluations and treatment goals and plans; provides correct information on Medicare, AHCCS and all other forms; regularly update s documentation and maintains all required prescriptions and signatures.
- Documents the course of patient care including progress made and continuing need for treatment; provides correct statistics and billing information on all PT patients; provides patient and family education on tasks and resources that will assist with continued functional independence
- This position provides, evaluates and documents wound care to patients.
- Collaborates and coordinates with other members of the health care team regarding patient treatment and discharge.
- Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families.
- In addition to clients in the Inpatient Rehabilitation Program, may serve clients including but not limited to the following: outpatient, hospitalized patients, and clients who are in the homebound or hospice settings.
- Provides skin assessment of patients on admission and subsequently in the PT process. Develops and implements wound care program to include patient care protocols, documentation tool and wound care formulary.
- Provides basic wound training and orientation to all new nursing and therapy staff. Provides ongoing in-services and training related to wound care, ostomies and incontinence. Is always a resource for staff and physicians.
Requirements:
- Doctoral Degree preferred.
- Must have graduated from an accredited Physical Therapy curriculum.
- Basic Life Support (BLS) within 60days of hire.
- Utah State license for Physical Therapist.
