Description
The Physical Therapist 1 is responsible for evaluating/assessing needs of referred patients and formulating treatment plans. Provides therapy services defined in treatment plans. Works cooperatively with physicians, case managers, and other members of the healthcare team. Maintains accurate documentation of therapy services in the medical record. May supervise physical therapy assistants, therapy techs, and students.
Responsibilities:
- Evaluation and treatment methods appropriate to diagnosis, age group, patient's physical and emotional status are performed and accurately documented and appropriate treatment plan is formulated as evidenced by chart review.
- Patient care is documented daily utilizing approved hospital and department documentation system reflecting patient response to treatment, actual performance, and treatment summaries if needed as evidenced by accurate & timely chart reviews.
- Daily patient charges are completed accurately, recording in treatment documentation, on daily charge sheet of department, as well as entering charges in appropriate billing system per unit 100% of the time.
- Measureable short and long term goals for each patient evaluated are identified and documented as evidence through chart review.
- Patient and family/caregiver education concerning treatment plan, expected reactions to treatment, safe use of equipment, home programs, and follow up care is accurately documented 100% of the time.
- Discharge notes that include a summary of the course of treatment, progress toward goals, discharge needs, recommendations for follow-up, and patient or family education are accurately completed 100% of the time as evidenced by chart review.
