Description
The Physical Therapist works directly with individuals to restore or improve physical abilities, promote behavioral changes, adapt surroundings, and/or teach new skills, to achieve optimal physical function and prevent disabilities, in accordance with principles and practices of the profession.
Responsibilities:
- Maintains licensure by obtaining required CEUs as determined by NYS Practice Act
- Adheres to professional standards, policies and procedures, federal, state and local requirements and Joint Commission (TJC) standards.
- Performs a comprehensive physical, psychosocial, and safety assessment as evidenced by clinical documentation. Identifies those functional problems that require physical therapy intervention based on an accurate assessment of the patients status.
- Must be capable of understanding and carrying out prescribed therapy services as requested by physicians and/or midlevel providers.
- Develops a plan of care to promote optimum function and safety, as documented in the clinical record.
- Completes all required documentation following evaluation, treatment and procedures and ensures that documentation reflects care provided and outcomes achieved.
Qualifications:
- 4 year/Bachelor's Degree in Physical Therapy from an accredited college or university, required.
- Master's Degree of DPT, preferred.
- Experience as a Physical Therapist prior to employment, preferred.
