Description
The Physical Therapist works directly with individuals to restore or improve physical abilities, promote behavioral changes, adapt surroundings, and/or teach new skills, to achieve optimal physical function and prevent disabilities, in accordance with principles and practices of the profession. The Physical Therapist may provide evaluation and treatment of patients in acute care, nursing home, short-term rehabilitation and/or outpatient settings.
Responsibilities:
- Maintains licensure by obtaining required CEUs as determined by NYS Practice Act
- Adheres to professional standards, policies and procedures, federal, state and local requirements and Joint Commission (TJC) standards.
- Performs a comprehensive physical, psychosocial, and safety assessment as evidenced by clinical documentation. Identifies those functional problems that require physical therapy intervention based on an accurate assessment of the patients status.
- Must be capable of understanding and carrying out prescribed therapy services as requested by physicians and/or mid-level providers.
- Develops a plan of care to promote optimum function and safety, as documented in the clinical record.
- Completes all required documentation following evaluation, treatment and procedures and ensures that documentation reflects care provided and outcomes achieved.
- Demonstrates the ability to evaluate and treat the assigned number of patients per day, and determine the need for follow up visits to maximize the functional outcomes.
Qualifications:
- 4 year/Bachelor's Degree in Physical Therapy from an accredited college or university, required.
- Masters of DPT, preferred.
- Experience as a Physical Therapist prior to employment, preferred.
