Assess referred patients, ranging in age from 18 years to geriatric who are experiencing rehabilitation diagnosis or conditions requiring rehabilitation. Provide direct patient care to assess patient medical condition, functional capabilities and limitations and restrictions and potential for rehabilitation. Establish and administer a treatment program with specific goals determined according to the patients’ capacity and tolerance under the direction of the Physician. Educates patients and families in appropriate physical therapy methods.
Duties and responsibilities:
Complete initial and regular patient assessment and reassessment in timely manner documenting outcomes and interpreting of data.
Document outcomes and interpretation of data. Communicate interpretation to Physician and interdisciplinary team.
Plan, coordinate, implement and participate in treatment plan as prescribed by physician and interdisciplinary team.
Administer appropriate physical agent; provide training, education, and fittings with patient and family members; report both oral and written treatment and status regarding patient progress and discharge.
Evaluate care interventions and their effect on outcome; document progress and variances to expected outcomes.
Ability to project a Professional Image.
Knowledge of regulatory standards and compliance requirements.
Strong organizational and analytical skills. Exceptional leadership abilities.
Working knowledge of personal computer and software applications used in job functions (Word processing, graphics, databases, spreadsheets, EHR, etc.)
Graduate of an accredited school for Physical Therapy.
Master's Degree, required.
A minimum of two (2) years’ experience directly related to duties and responsibilities, preferred.
In-patient rehab experience preferred.
Certificates, Licenses, Registrations:
Physical Therapy licensure, in good standing, within and by the state of work.
Current BLS certification that must remain current throughout employment.
