Description
Under the direction of the PT team lead, provides physical therapy services to patients following the plan of care established by a Physical Therapist.
Essential Functions:
- Provides physician prescribed physical therapy under a plan of care established by the PT.
- Instructs client and caregiver in treatment programs, precautions, and equipment use as necessary and documents client/caregiver demonstration of instructions provided.
- Prepares medical records, progress notes and updates care plans for each patient visit in a timely manner as per Agency policy.
- Is responsible to work within the guidelines and authorizations provided by the individual's insurance coverage, AND/OR Medicare guidelines.
- Participates with all other health care personnel in patient care planning.
- Performs all skilled procedures as ordered by physician and according to the plan of care established by the PT.
- Consults with PT regarding change in treatment.
- Participates in Quality Assurance and Performance Improvement activities as assigned
- Attends all patient care conferences as scheduled.
Qualifications:
- Associates or better in Health Services Administration.
- Certification CPR.
- Lic Physical Therapy Asst.
