Description
Primary Responsibilities:
- Provide physical therapy services to patient according to physician’s plan of care. This may include, but will not be limited to assessing and evaluating therapeutic / rehabilitative / functional status, participation in the development of the total plan of care and evaluates home environment.
- Initiates physical therapy program and instructs other personnel and / or family caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
- Prepares and submits clinical and progress summaries in a timely manner.
- Participates in discharge planning for patient.
- Supervises physical therapy assistants according to organization policy and state regulations, treat and re-evaluation patient no less than every 7th day of treatment or 21 days whichever occurs first.
- Maintains contact / communication with other personnel involved in the patient’s care to promote coordinated, efficient care.
- Maintain clinical competency in physical therapy practice and theory.
- Will participate in Agency on-call coverage as outlined in Agency policy.
Qualifications:
- Graduate Degree from an Accredited Physical Therapy Program.
- Active and unrestricted Nevada Physical Therapist license.
- CPR certification or obtain within 30 days of hire.
- 2+ years of experience as a Physical Therapist.
- Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area.
