Physical Therapist, Home Health

Responsibilities:

• Provide Physical Therapy services to patients in their homes, assessing patient functioning levels, and developing treatment programs to improve independence and physical/cognitive functioning.
• Be involved in patient and caregiver education, supervision, and reporting on patient progress.
• Perform Physical Therapy assessments and develop treatment programs.
• Record and report patient's reaction to treatment and condition changes.
• Instruct patients and caregivers in therapy programs and device use.
• Supervise physical therapy assistant and, when appropriate, the certified nursing assistant.
• Prepare reports and participate in staff and Agency development activities.

Qualifications:

• Active PT state license (IL state).
• Master's degree.
• 1 yr experience in healthcare setting.
