Description
Physical therapist position in new Outpatient Spine Wellness center; this therapist will be part of a multidisciplinary team of medical, rehabilitation, and wellness professionals dedicated to improving the health of all patients. This therapist will collaborate with professionals throughout the facility who provide rehabilitation, pain management, physical medicine, integrative medicine, fitness, nutrition, and behavioral health services to improve surgical and non-surgical outcomes.
Knowledge, Skills and Ability Requirements:
Computer proficiency in Microsoft Word, Excel, PowerPoint as well as electronic health records and other software applications.
Ability to establish and maintain relationships with referral sources.
Effectively manages patient case load in a fast-paced, collaborative environment.
Ability to solve problems and work independently with minimal direction.
Strong leadership, mentoring, and motivational skills.
Strong sense of professionalism and proficient interpersonal, clinical, and communication skills.
Commitment to quality, evidenced based care, and client experience.
Responsibilities:
Performs evaluations for strength, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function, endurance, gait and functional abilities in activities of daily living through physician referrals. Recognizes physiological, psychological, pathological, and age related changes in the patient; consults with other Health Care Team members, and establishes measurable goals with the patient/family based on appropriate patient care protocols and critical paths in order to meet patient needs.
Plans, implements, delivers and delegates patient care by utilizing appropriate interventions, such as heat, cold, light, water, sound, electricity, mobilization, manual therapy, and therapeutic exercise based on assessments, revising treatment when needed, instructing patient/family through verbal, written and audiovisual materials and participating in discharge planning including recommendations to community resources to meet patient needs throughout the continuum of care.
