Description
Responsibilities:
- Completes thorough and accurate patient assessments utilizing physical therapy assessment skills and interviewing techniques.
- Develops and coordinates treatment protocols/programs for patients referred to the Rehab Services Department by referring physicians.
- Plans appropriate therapy interventions based on individual patient needs.
- Interventions are carried out using proper technique, procedure, and safety precautions to meet individual needs of the patient.
- Evaluates care provided for effectiveness and continuity. Make necessary changes in treatment protocols/plan as indicated.
- Consults with physicians and nurses regarding any change in the patient’s condition or treatment programs.
- Includes patient and family/caregiver in planning of care and educates family/caregiver on proper techniques in caring for or assisting the patient.
- Thoroughly, accurately, and in a timely manner, documents assessments, interventions, care given, education provided, patient response, and effectiveness of care provided.
- Measures and fits patient supports and appliances accurately. Trains patients and family in care and use of supports and appliances.
Qualifications:
- Requires specialized knowledge as to administration of treatments and maintenance of equipment.
- Must exercise sound judgment in administering and analyzing treatments.
- Requires a Bachelor of Science degree from an AMA approved and accredited school of Physical Therapy.
- Requires successful completion of state board examination and Indiana licensure.
