A License Physical Therapist with a minimum of 2 years of clinical experience in an out-patient or ambulatory care setting. With ability to perform assigned task independently and/or with minimal supervision. Responsible for evaluating physical therapy needs and implementing comprehensive and individualized treatment to assigned patients. Supervises Physical Therapy Assistant as assigned by the director.
Responsibilities:
- Perform evaluation and treatment. Utilizing evaluation/re-evaluation findings, chooses appropriate equipment, therapeutic techniques and/or modalities to achieve optimum individualized patient results.
- Prepare daily work schedule allocating adequate time for completion of assignments.
- Provide treatment that include but is not limited to therapeutic exercises, transfers, bed mobility, ambulation, neuromuscular re-education, pain management with documentation in progress note.
- Review patient’s medical chart prior to initial contact and subsequently at periodic intervals to obtain pertinent information and consult with Physician and/or appropriate members of the interdisciplinary team.
- Perform patient evaluation/re-evaluation which may include goniometry, manual muscle testing, transfers, bed mobility, ambulation, skin condition, sensory testing, standardize testing and document appropriately.
- Interpret evaluations/re-evaluation findings and formulates individual treatment programs with time frames for short-term and long-term goals and record progress on EMR.
- Establish treatment programs for individual patients.
- Maintain proper and timely records concerning patient treatment, attendance and progress as well as short and long-term goals.
- Assign treatment procedures and supervise progress of patients treated by PT Assistants.
- Valid license and current registration to practice as a Physical Therapist issued by the New York State Education Department (NYSED); or,
- Valid limited permit to practice as a Physical Therapist issued by NYSED.
