Job Overview
Responsibilities:
- Perform initial and ongoing physical therapy evaluations according to company policy and procedures.
- Assess, develop and implement a plan of care to promote optimum function and safety.
- Develop a treatment plan for each patient based on initial and ongoing evaluations including short and long-term goals, collaboration demands of other staff members which will require patient and caregiver involvement.
- Document patient progress according to approved medical documentation and facility documentation procedures.
- Facilitate communication and serves as a liaison between and among nursing and direct staff, family and/or guardians, and community medical providers and professional consultants in cooperation with operations management.
Requirements:
- Valid license as a Physical Therapist in the state of Minnesota.
- Excellent interpersonal skills, including the ability to work respectfully with a diverse range of individuals in a professional manner.
- Working knowledge of Microsoft Office suite.
- Ability to read, write and speak fluently in English.
- Must provide valid license, and proof of insurance.
- Prefer two-years prior home health experience.
