Responsibilities:

1. Provide therapy services to patients in skilled nursing facilities, independent living, and assisted living facilities.
2. Monitor patients’ progress and document their responses to treatment.
3. Collaborate with a multidisciplinary healthcare team to provide comprehensive patient care.
4. Educate patients and their families on treatment plans and home exercise programs.
5. Conduct assessments and develop individualized treatment plans.

Qualifications:

1. Licensed Physical Therapist.
2. Strong interpersonal and communication skills.
3. Ability to work independently and as part of a multidisciplinary team.
4. Flexibility to travel between different facilities and patient homes.
5. Ideal candidate location would be the Sugarland/Missouri City area.
6. This will start as a PRN position and will grow to Full-Time as we expand our footprint and caseload within the buildings.
