Job Overview
Responsibilities:
- Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of Physical Therapy.
- Responsible for providing the appropriate level of clinical direction for Physical Therapy Assistants.
- Plans, implements, manages and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patient's needs are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines.
- Manages the activities of the multi-disciplinary team providing care to the patient.
- Monitors the provision of therapeutic services through written and verbal communication with all disciplines.
- Develops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team.
- Interprets agency policy to patients/families.
- Reviews patients visits weekly and provides plan to PCS and team assistant, including anticipated weekend visits.
Certifications and Licensures:
- BLS (Basic Life Support).
- PT (Physical Therapist) License.
- Driver's License State of Florida (Florida driver's license is required within 30 days of hire).
